New Patient Registrationregister_now

To register with the practice you should attend in person and you will be asked to complete a registration form (GPR) which incorporates essential data required for obtaining your previous medical records. You will also receive a health questionnaire to complete which gives us some information about past/present medical history. We also require proof of address and ID to complete your registration.

If you are not a British Citizen we will require you to bring your passport. If you are a student, or are working in the UK, we will require documentary proof of this - e.g acceptance letter/wage slip. Please note that you may be charged for your consultations and prescriptions based on the information that you give us. This will be verified once your details have been checked.

An appointment for a new patient medical will be arranged with the practice nurse, to which you are asked to bring along a sample of urine.

A doctor will check all the information you have supplied and decide if your registration will be accepted. If, for any reason, your application is declined (this is a very rare occurrence) we will contact you to explain the reason.

Your registration will be with the practice rather than with a particular doctor.
